All members, life and associate, planning to attend the 2022 National Veterans Wheelchair Games in Tempe, Arizona and who want the chapter to pay for transportation and lodging must submit both pages of this chapter sponsorship form to Keystone PVA, 1113 Main Street, Pittsburgh, PA 15215.
ALL SPONSORSHIP FORMS MUST BE POSTMARKED NO LATER THAN 31 DECEMBER 2021. THERE WILL BE NO EXCEPTIONS.
